Eligibility:

• Age 35-40:  only one mammogram allowed during this timeframe
• Age 40+:  one mammogram allowed per year
• Last mammogram has been more than one (1) year/12 full month  (On or before September 28, 2018)

• Must be uninsured or underinsured (if patient is insured but does not have coverage for mammograms or co pay and/or deductible is something they cannot afford for the service then they ARE eligible to receive a free mammogram ($500 Co-Pay or deductible considered underinsured).

• If the woman is
• experiencing lumps, bumps, or discharge in their breasts
• has breast implants
• or they cannot stand on their own during the exam
• then they need to be referred to the Breast Health Center at 214.266.3302 for a diagnostic mammogram.

Documentation

• Must provide proof of Dallas County residency;

• Picture ID required with current address (work or government [US or foreign] issued)

OR

• Picture ID required AND a utility bill or other official business mail with current address and their name

NOTE: If patient does not have a bill in their name, (i.e. they live with someone else or the bill in spouse’s name) they can fill out a Residency Affidavit (a form we will have available on the day of the event). Patient needs to be notified to bring the person they live with to the event so that they can sign the form.

The health fair will be FREE and open to all – vendors and participants.
